Video Shoot Request Form

If you'd like to have a video shot for your event, ministry, or life-change story, please fill in the form below. Please include all information on the form. A lead time of SIX WEEKS is required for all video requests. Anything requested in shorter than SIX WEEKS will not be considered, or completion time will be adjusted.

All videos will be a collaboration between the Creative Team and the Person(s) requesting the video. You will receive an email response to get further information and plan filming and anything else that needs to be scheduled. All scheduling will be at the discretion of the Creative Team, and every effort will be made to accommodate schedules and other considerations.
